A meta-analysis of telehealth studies found that, compared to traditional hospital/clinic visits, those patients with heart failure receiving telehealth had a 42 percent reduction in re-hospitalizations. This study concluded that Remote Patient Monitoring has the potential to prevent between 460,000 and 627,000 heart failure related hospital readmissions each year.
Another study that focused on the impact of telehealth on hospital readmissions in general, found there was a 60 percent reduction in readmissions when Remote Patient Monitoring was used, compared with traditional hospital/clinic visits. When compared with other disease management programs that did not use Remote Patient Monitoring, a 50 percent reduction in readmissions was observed.
Examples:
- Stabilizing, following, and managing individuals with chronic conditions after an
acute episode (e.g., emergency room, hospital, or physician visit)
- Monitoring at home shortens hospital stays and reduces cost.
- Reinforcing hospital discharge plan instructions
